What is Infrapatellar Impingement?

The infrapatellar fat pad is soft tissue that lies behind and just below your patella. It separates the patella from the femoral condyle.

The infrapatellar fat pad distributes pressure across the patellofemoral joint, facilitates flexible deformation in the knee and supports patellar stability. This fat pad can become pinched (impinged) causing pain when moving.

Causes

What are the causes of Infrapatellar Impingement?

  • Overload of the quadriceps – such as with running or kicking
  • Repetitive hyperextension of the knee – bending too far backward
  • Repeated episodes of kneeling
  • Direct trauma to the front of the knee
  • Poor biomechanics i.e. flat foot
Symptoms

What are the symptoms of Infrapatellar Impingement?

However, infrapatellar inflammation can occur due to other associated knee conditions:

  • Sharp pain located at the front of the knee
  • Swelling surrounding the patella and the patellar tendon
  • Pain with prolonged periods of standing or sitting with crossed legs or after periods of rest
  • Osteoarthritis
  • Patellofemoral pain
  • Ligamentous injuries
  • Meniscal tears
  • Patella tendinopathy
  • Post-operative pain following an arthroscopy/keyhole surgery
