What is Extensor Tendinopathy?

Extensor tendinopathy is the failed healing process of the extensor tendons that run along the top of the foot. The tendon has an increased rate of matrix remodeling, leading to a mechanically less stable tendon that is more susceptible to damage.  Extensor tendinopathy is usually caused by repetitive motions that build up and irritate your tendon. Extensor tendinopathy is common in runners, especially if your shoes fit poorly or are laced up too tight.

Interestingly the same change in an overloaded tendon can also be found in tendons that are chronically underloaded.

Causes

What are the causes?

The general cause of a tendinopathy is overuse.

  • Tendons rubbing up against your shoes
  • Laces done up too tight
  • Overuse of the extensor tendons – causes micro trauma, which the normal ability to heal is slow
Symptoms

What are the symptoms of Extensor Tendinopathy?

Symptoms are likely to develop over time but involve:

  • Pain on the top of the foot
  • Initially pain present at the beginning of activity that resolves once you warm up and reappears after cooling down
  • Aching pain, which increases with exercise
  • Swelling on top of the foot
  • Curling the toes may trigger pain
  • Can usually pinpoint the site of pain

  • Wearing well fitting shoes
  • Anti-inflammatories if approved by a healthcare provider
